Your Best Option to Accessing Expert Dementia Care at Home

Our NaviGuide and care team members will provide clients and caregivers with in-home education, resources and support to navigate the challenges Alzheimer’s disease and dementia may present over time.

The Centers for Medicare and Medicaid Services (CMS) launched the GUIDE program. Medicare’s new Guiding an Improved Dementia Experience (GUIDE) model offers comprehensive care and service coordination for persons living with Dementia  and their caregivers, as well as access to in-home or facility-based respite care. All of this is available at no cost to eligible Medicare beneficiaries.

ELIGIBLE CLIENT BENEFICIARY
– Traditional Medicare beneficiaries with a confirmed dementia diagnosis (including Alzheimer’s, Lewy Body Dementia, Frontotemporal Dementia, Vascular
Dementia, etc.)
– Living at home or in the community

NOT ELIGIBLE
– Medicare Advantage enrollees
– Living in nursing homes
– Participating in Hospice or a PACE organization

Your dedicated in-home senior health care Service Coordinator can help you with a range of common issues that modern seniors face when in need of more daily assistance and support.

Common issues we help you address include:

  • Coordinating and scheduling appointments
  • Arranging for transportation, chore and errand services
  • Information and connections to various community programs
  • Securing help at home from personal care aides and home care services

Your Service Coordinator can also support you with navigating common concerns, like:

  • Having trouble keeping physician appointments because of transportation challenges
  • Needing extra support to feel comfortable going out to attend events or run errands
  • Having trouble navigating the healthcare system

Your Service Coordinator will take the time to get to know you and provide customized solutions for your specific situation and needs.
