NaviGuide is an affordable option for aging adults who don’t want to give up their independence. Your NaviGuide journey begins with an initial in-home visit. The service coordinator will explain the program, answer questions, and conduct an in-depth interview to determine the ways NaviGuide can help seniors live their best lives.
After the in-home initial assessment the service coordinator will partner with your family to develop a personalized, flexible plan for support that changes as wants and needs evolve.
You can choose to sign up for NaviGuide membership without the initial in-home assessment, if desired.
